PUBLIC NOTICE OF HEALTH CARE FACILITY CLOSURE

Effective as of 11:59 PM on November 1, 2026 (the “Closure Date”), SJ East Campus, LLC d/b/a Denver Surgery Center (the “Company”) will cease operating Denver Surgery Center, an ambulatory surgery center located at 1830 Franklin Street, #200, Denver, CO 80218. Following the Closure Date, the Company will no longer participate in the Medicare Program (Title XVIII of the Social Security Act), and the related agreements with the Secretary of the United States Department of Health and Human Services will be terminated in accordance with the provisions of the Social Security Act. The Medicare Program will not make payment for services furnished to patients after the Closure Date.
